The Heart Failure Center is composed of multidisciplinary medical care personnel, aiming at preventing recurrence and aggravation of heart failure and improving the patients’ quality of life.
The activities of the Heart Failure Center include inpatient/outpatient rehabilitation, patient education through lectures, and lifestyle guidance using our established notebook for heart failure management.
The Heat Failure Center holds case conferences to discuss and improve the team approaches to patients, and hosts workshops to improve the community healthcare.
